3. Parents' names -- please include whether you wish it to say only first names or for it to include their last names as well (i.e., son of Lisa and Mark or Lisa and Mark Stein or Lisa Cohen and Mark Stein -- whichever way you wish it to appear).
4. You may also choose to include the parents' Hebrew names. You may, however, choose to only include their English names, if you prefer.
